Fullmind Educator: How to Review & Update IEP Goals
SWD Progress Monitoring · Fullmind Educator Guide · August 2026
Reviewing a student's IEP and documenting progress against each IEP goal in Progress Monitoring is how Fullmind produces defensible, review-ready evidence of the instruction you deliver. As a Fullmind Educator assigned as a responsible educator, you review the student's full IEP, accommodations, and goals, then log a progress note against each goal you're responsible for at the prescribed cadence — recording the check-in date, a progress status, and a narrative note with supporting data. Each entry becomes part of the student's record for school and Fullmind admin review, so the documentation stays current and complete against every goal — the evidence captured as you teach, rather than reconstructed after the fact.

Log into Fullmind LMS
- Log in at https://lms.fullmindlearning.com/login.
- Navigate to the courses tab and select the Course ID link for the course requiring progress monitoring.
Find the student & open Progress Monitoring
- From the Course Page, select a student requiring progress monitoring
- Select the student on the roster section
- On the student page, navigate to the “Progress Monitoring” tab
Review the IEP, Accommodations/Notes, & IEP Goals
-
When available, review the "Notes and Accommodations" section on the student's Progress Monitoring tab. Although these notes offer valuable context, educators must still examine the full IEP, as the notes do not completely capture all requirements specified in the official documentation.
-
To review the IEP, select the file uploaded under “Documentation” This will open the PDF reader, which you can now review the full IEP.
-
To review the IEP Goals, you can navigate to the “Goals Overview” section. Goals are visible as an overview or individual to support areas (i.e., math, reading, SEL, speech, etc.). The “Responsible Educator” is the educator assigned to update progress notes at the prescribed cadence.

Updating IEP Progress Notes
-
Navigate to the subject or support area tab, when ready to update progress note, select update status. This will open a popup window.
- Hint: Educators can update notes early at their convenience; the next due date will adjust based on the most recent update.
- In the Update progress pop up, you will see the goal, criteria, and method of measurement if details were clearly noted in the student’s IEP. If details are missing, the educator will use their expertise to determine appropriate criteria or methods.
-
Select the Check-in date (Date of Note) and Progress Status from the dropdowns, then add a narrative note with data. Review and save to log the note for school and Fullmind admin review (work sample uploads coming soon).

Narrative Notes Criteria Give (task or condition)__________________ (Student Name)___________ is able to (restate goal) _______________________ with______% accuracy, retaining the skill for ____ weeks with/without review/reteaching. If “Making Limited Progress,” or “Not Making Progress,” include a note regarding the barrier or challenge and next steps (example: The primary barrier this period was ___; next period will incorporate ___.) If “If Insufficient Data to Measure,” indicate cause note and plan for next period. |
Example Progress Notes
-
On Track to Meet Goal
- Given a grade-level nonfiction passage, the student read 82 words per minute with 77% accuracy, measured by running record. This is on pace to meet the goal criterion of 90 WPM at 90% accuracy by the IEP end date. Errors clustered on multi-syllable words. Running record attached.
-
Making Limited Progress
- Given a monthly district writing rubric (4-point scale) applied to a multi-paragraph writing sample, the student scored 2 of 4 on 2 of 4 samples this period. This is not yet on pace to meet the criterion of 3 of 4 on 3 of 4 consecutive samples by the IEP end date. The primary barrier was topic-sentence structure; next period will incorporate a graphic organizer and modeled topic sentences. Writing sample attached.
-
Not Making Progress
- Across three probes this period, the student produced 1 of 4 rubric scores at target, unchanged from the prior period. This is not on pace to meet the goal by the IEP end date. The barrier appears to be [decoding / attention / prerequisite skill]; next period will [specific adjustment]. If the trend continues, this goal may warrant IEP-team discussion.
-
Insufficient Data to Measure
- No progress data was collected this period because all scheduled sessions in the collection window were cancelled by the school. Measurement will resume at the next scheduled session.
